Sun Life insurance card. This allows us to continue to treat our patients to the fullest extent while
alleviating the majority of the financial investment of dental services.
All CDCP patients of Eastman Dental will be responsible for paying any and all potential remaining
amounts directly to our office. Sun Life will never reimburse a CDCP patient and will only reimburse
providers for services covered under the CDCP.
Before attending your dental appointment, we will require the following proof in order to provide
services:
1. Confirm patient eligibility for the CDCP
2. Confirm the patient is covered for select services and at what percentage
